Provider Lookup

Below are a list of link(s) to provider lookup utilities. Select the plan that you are interested in and you will be directed to the appropriate site. On each carrier's site are specific instructions to assist you in making the proper choices to find a provider. Please contact member services if you need any assistance in locating a provider.

Health Net HMO

Find an in-network provider

Log on to https://www.healthnet.com/ and select "Search Our Doctor Network" on the right-hand side. When prompted to "choose a plan" select "HMO Group". When you have located the provider you wish to choose, click on that provider name. A list of medical groups that your provider is associated with, will be listed along with a number for each medical group. Choose the medical group you wish to belong to and write that number along with your doctor's name on your application:

Health Net PPO - California Residence

Find an in-network provider at https://www.healthnet.com/ and select "Search Our Doctor Network" on the right-hand side. When prompted to "choose a plan" select "PPO Group"

Health Net PPO - Non-California Residence

Find an in-network provider at HealthNet - Select "Doctor Search" on the left-hand side and follow the steps.

DeltaCare

Under "Find a Dentist" select the DeltaCare USA network

Delta Preferred

Under "Find a Dentist" select the Delta Dental PPO network

Landmark

Select "Member" on the menu and then select "Provider Directory"

Group PLan is "All Others"

MES Vision

Select "Group Provider Search" MES Vision
