Medical Benefits
Professional Musicians Local 47 and Employers Health and Welfare Fund offers two medical benefit options for eligible participants, and their dependents, who reside in California. The Trust provides a separate medical benefit option for eligible participants who do not reside in California, but in one of the other contiguous States.
California Residents
California residents have a choice. Health Net provides a Preferred Provider Organization (PPO) and a Health Maintenance Organization (HMO). Review and compare each plan carefully as the benefits, out-of-pocket expenses and premium rates are different for each plan. Participants enrolled in either the Preferred Provider Organization (PPO) or Health Maintenance Organization (HMO) are also enrolled in Landmark Healthcare's chiropractic and acupuncture benefit. Non-California Residents
Health Net provides a Preferred Provider Organization (PPO) benefit, for NON-California residences

Complentary Benefits
Professional Musicians Local 47 and Employers Health and Welfare Fund provides a Chiropractic and Acupuncture benefit through Landmark Healthcare. This benefit is at no additional cost to the participant and their enrolled dependents.

Theis benefit is available to California Residence ONLY.
Life and Accidental Death and Disability
Prudential Financial underwrites life and Accidental Death and Dismemberment coverage. This benefit covers the participant only. Dependents are not eligible for this benefit.
