Welcome to the Professional Musicians Local 47 and Employers Health and Welfare Plan (MHW). This site has been established to inform Professional Musicians of the availability of health care through a Plan sponsored by Professional Musicians, Local 47. Information regarding benefits, eligibility, enrollment are provided by selecting the resource links to the left, or by downloading the Trust's SPD which provides basic information concerning the benefits available to you through Professional Musicians, Local 47 and Employers' Health and Welfare Fund.
The MHW Trust provides medical, dental, vision, and life insurance to musicians who qualify for benefits based upon contribution received on their behalf in accordance with Professional Musicians, Local 47 and American Federation of Musicians contracts and collective bargaining agreements.
Changes to the member co-premium and eligibility amount were made effective October 1, 2008. Please review the Summary of Material Modification for complete details of the changes.
The program is administered by PacFed Benefit Administrators (PacFed). If you have questions, you may contact PacFed directly Monday through Friday, 8:30 a.m. to 5:00 p.m.
Benefits
Currently the Fund's providers are Health Net, Landmark Health Care ("Landmark"), Delta Dental, Gerber Life, Prudential Financial.
Detailed information about Life and Accidental Death and Dismemberment benefits (provided through Prudential Financial), Medical benefits (provided through Health Net), Dental benefits (provided through Delta Dental of California and Delta Care PMI), Chiropractic/Acupuncture benefits (provided through Landmark Healthcare) and Vision benefits (provided through Geber Life and administered by MES Vision) are described in summary through this SPD and a more detailed description of those benefits are set forth in separate booklets for each benefit which will be issued at the time of your enrollment for those benefits.
Health Net offers a Preferred Provider Organization (PPO) Plan for Participants residing outside the State of California. Proof of residence is required to enroll in this plan. Chiropractic, Acupuncture, Dental and Vision benefits are not available to those residing outside the State of California.
